Prices seem to be fluctuating frequently as Amazon keeps cutting the price and Chewy eventually does the same. You may see $48.86, which is still a pretty darn good deal.
This is the lowest price per application I've ever seen on this product from a reputable seller. I checked camelcamelcamel also and this is the lowest price they show in their amazon price history.

So, the medicine are all the same but the size of applicator and volume are different.
If your dog is 10lb, for example, you can buy one for medium dog (0.045oz per applicator), and then apply a half of it (0.0225oz). Or even you can buy the large dog one and apply 1/4 of it. This way you can save a lot of money if you have smaller dog.
Some people measure and apply this with 0.001 oz/lb to their pets as well. If your pet is 10lb, you can just use 0.01oz per application.
I have been doing this to my dog (11lb) with no problem. Of course, vet and manufacturer do not recommend this. And you will have to do this with your own risk
