I did the port-in at costco. The printed rebate form has both the temporary # and the ported # on it.

I will be using the ported # for all the rebates.

The instructions (step 5i) say to use "Permanent wireless number. Do not use a temporary number." The number printed on the form is a temporary number, and you know your permanent number.

Thanks, when I tried to send the submission, I got the message that " We already have a rebate submitted for this Control Number". Is it possible that he gave same paper to someone else too, since info was filled by him using hand?

I don't think it's worth adding Kickback on these free lines. If you have hook-up and stuff, you barely save 6-8$ overall.
