Porting in from various networks: don't use Google Voice, Boost or any MVNO that uses Tmobile as their network. Even if the rep let's you do it, your rebate will get rejected later.
It is not known if you can stack the costco $200 with the tmobile $100 but one thing to note is that the costco rebate is by Wireless Associates which runs the booths in Costco, while the $100 is directly by T-mobile.

If you do it yourself online, you'll get charged $10 for the SIM card. If you go in store, you'll get charged $10 for the SIM card and $20 for an assist fee. If you call or message, you'll get charged $10 for a SIM card and $20 for an assist fee but there's half a chance the rep will waive one or both of those if you ask nicely (though not guaranteed).
