Porting in from various networks: don't use Google Voice, Boost or any MVNO that uses Tmobile as their network. Even if the rep let's you do it, your rebate will get rejected later.
It is not known if you can stack the costco $200 with the tmobile $100 but one thing to note is that the costco rebate is by Wireless Associates which runs the booths in Costco, while the $100 is directly by T-mobile.

My understanding is that for Costco's promo, I will be keeping it with the BYOD's device IMEI for at least 3 billing cycles to qualify for the rebate.
The port-in rebate only requires to be on newly activated line and eligible plan and port from an eligible carrier. No other mentions or requirements on the devices.
The Pixel promo only says to require a newly activated line and trade in to receive the credits and does not mention requiring the device to be active on the new line. (The rep also told me I can use the device on other lines and the statement credit / promo will be tied to the new line as long as I make the purchase from that line)
So in the end, I'm taking a risk and hoping to receive 1. Costco $200 cash card, 2. TMO $100 rebate, and 3. Free Pixel 4a 5g after 24 months statement credits.
