Only the Platinum card. Also this is in-house cell phone insurance by Amex, Visa and Mastercard's cell phone insurance is administered 3rd party and they are a pain to deal with
To get coverage:
You must charge your monthly Eligible Cellular Wireless Telephone bill to your Eligible Card Account. You are eligible for coverage the first day of the calendar month following the payment of your Eligible Cellular Wireless Telephone bill using your Eligible Card Account. If you pay an Eligible Cellular Wireless Telephone bill with your Eligible Card Account and fail to pay a subsequent bill using your Eligible Card Account in a particular month, your coverage period changes as follows:
Seems to be AIG subsidiary, "Coverage is provided by New Hampshire Insurance Company, an AIG Company."

My bank offers a 600 dollar cellphone protection thing too. I guess it's getting pretty popular to pull people in.

also it says you have to pay your bill with it. you will lose your 10 dollar discount for auto pay won't you for using your credit card instead of bank account/debt?

The coverage doesn't cover if I purchased the phone from apple and am getting the service from T-Mobile, correct? The terms were saying:
"Eligible Person" means a person to whom an Eligible Card Account is issued from a Participating Organization who charges the monthly bill for an Eligible Cellular Wireless Telephone to an Eligible Card Account.
Too many eligibles in one sentence.

I'm in a Tmobile family plan with my parents. The account is technically under my dad's name. If I was to pay for our monthly bill using my Amex card, would we be eligible for the insurance?

I pay with my Amex and still receive my autopay discount with T-Mobile.

450 dollars a year for this card. It seemed to be good at one point but hf how disappointing this card has become. Can you imagine the company hasn't fired the manager in charge of rewards yet?! It's just amazing

I make more than the annual fee back every year even without traveling as much as I used to. One of my fave cards of all time.

so lost phones are not covered?

If you lose it just say it was a "involuntary departure" had it happen to me look up what that means. Just don't say you lost it or it was stolen to make it easy say it was involuntary departure like it was dropped in water I know where it is but I don't have the ability to get to it.
