I mentioned the offer to my wife who is a Chase employee and she pulled it up on her internal system. It had a note that they had additional offers if you apply in a branch. You get the 100k points, 1st year AF waived, and a $50 grocery credit. So essentially an extra $145 for applying in the branch.
Wow. No brainer if you are eligible for the bonus.

Is there a way to find out date of last bonus I received? -- I had the CSP & CSR when they first came out & then i cancelled after a year -- dont know if it has been 48 months since the last bonus.

If you had them when they first came out then you should be ok, but you can check your old card statements online to determine when you received your bonus to your account.
I applied for credit at branch on Tuesday, that card still did not come yet . I call Customer service on Thursday evening, I got the card on Friday noon . I used 1-800-436-7970 .

Same situation with you, clueless rep, but once I got approved and she printed out my paperworks, it's all written there, 100k, waived AF and $50 grocery credit. Applied on the 25th and still no card in the mail.
I did on 26th, haven't got the card as of yesterday. Did you get yours?
yes, as long as the account is open you'll still get extended warranty coverage, even after it's converted to freedom. moreover, freedom offers the same extended warranty benefit anyway.
also, you should not downgrade in 11 months, and wait at least 12 months from account opening date. you'll get 30 days from when the second annual fee posts to convert it to freedom and qualify for a full refund. past that, you still get a prorated refund.
What's the downside of downgrading in 11 months? Is there a condition in the terms that you have to keep the card active for 12 months to get 100k points? I thought 100k points are ready to spend once you spend 4k in 3 months.
Bonus offer changes from time to time. Credit card welcome package is always generic and does not include the current offer info. Such info should have been disclosed on your application when you signed up. You may download the Chase app. The spending tracker will show up there a couple days after your card is activated.

Does that mean that I can apply for the CSP 3 days after downgrade?
If you downgrade to a non Sapphire product you should generally wait 5-7 business days, but some have had luck after 3 days.
