Your mail flyer offer is probably the same as the branch offer which is open to everyone to apply for the credit card via branch banker which is how I got my $300 bonus.
What Browser r u using? The application form is blank when I use chrome, I don't know what it is asking!

Not sure if someone's mentioned it but this card is only good if you plan to spend less than 2k a month...anything more than that in a given month you are better off getting the Citi double cash card
I tried product change from Double Cash to this but they couldn't do it.
It may vary by card, but that was what I heard others say when they pulled the plug on our Forward cards (really hated to see that one go). Nobody could get them to switch products then, either.

If memory serves, the application form was a popup page. I have a popup blocker and it asked me if I wanted to allow it. Turn off all your Chrome extensions and try again.
Not sure if someone's mentioned it but this card is only good if you plan to spend less than 2k a month...anything more than that in a given month you are better off getting the Citi double cash card
Why not get both. With the Custom Cash card, get 5% for $500 spent in one category and everything else with Double Cash card. You will get extra 3% back for up to $500 spent in the one category. If you plan to have only one credit card, Citi Double Cash with flat 2% or other flat 2% cards would be the way to go. See my post above. There are various combination of cards to maximize you cash back return on purchases.

Yup, that was what I did. Took about 10 mins since I already have Citibank account. $300 after spending $1000 in 3 months. Branch banker told me that there is no way to get $300 bonus applying online. It is an in branch offer.
Got my card and now looking to buy stuff to get the $300 bonus (applied in branch so the deal is spend $1000 get $300 bonus). Does anyone know what the percentage back / spending category would be if I only want to make a single purchase of a $1,000 Costco Shop card (Costco online supports MasterCard purchases of their Shop card, for members only, then that can be used to buy anything at Costco, and members get the standard 2% back to apply to membership fees annually using the Shop card - if at checkout you present your membership card then pay with their Shop card only) with this Citi card, to gain the ultimate $300 bonus? I kinda just want to do one large purchase like that as I don't have anything else I really need to but at the moment to get the deal completed. I know I can buy the one Costco Shop card, but my real question is what percentage back on the Citi card will I receive or the purchase of that one item?

I think I will do it that way, $500 Costco Shop/gift card (which will be as you said 1% cash back, 500 points) this month and then $500 in groceries (too bad Costco isn't included in the grocery category, as that is where I buy most groceries) next month. I have to do it that way as I only got my card this week and there is not enough time to use it for anything besides an online purchase like this gift card. You only get 1% for anything outside their categories, like most online orders such as Amazon, Walmart, etc. I wish online store purchases or wholesale clubs were a category, but I see why they wouldn't do that. Should be 1% cash back if so for the one shop card item from Costco this month and then next month 5% back on the groceries. Then I will be all done with the spend $1000 in the first 90 days to get $300 back.
