No, not all credit card applications are hard pulls. Amex many times doesn't do a hard pull if you already have a card from them.

It would be either groceries or gas, or whatever the one single category you spent the most on that month. Categories won't rotate like other cards. The verbiage:
Earn 5% cash back on purchases in your top eligible spend category each billing cycle, up to the first $500 spent, 1% cash back thereafter.

It rotates monthly according to your spending. The highest category in the month will return %5. Others will return less

When I got my bonus at the branch, the banker told me there was no end date she was aware of. My two Citibank branches had brochures on display about the $300 bonus in branch only.

No you dont get to choose. It will automatically apply to the highest spend category each billing cycle.

EDIT: I can actually see some use for this for someone who doesn't spend too much in those categories but wants to maximize their cash back. Especially if you use mobile payments and don't have to carry the card in your wallet.
