First off, I am a Costco guy myself and wouldn't pay for a Sam's Club membership.
If I get a $45 gift card for a $45 membership - it's FREE. I don't understand the debate.
On top of it, Amex Offer has been pretty reliable for me. And if it says $25 statement credit is on the table, I don't have reason not to believe it.
Update 2: data points suggest this deal does stack. Many users reported getting both Sam's gift card and Amex credit. It does take a loooong time, 5 weeks for the sam's gift card and 30 days for amex credit for me personally.

Chatted with Sam's Club today. Told him that I hadn't received the eGift card after more than a month. He took a few minutes and confirmed I should have received it by now as my membership purchase qualified. Stated I would be sent a new one in 3-5 days.
Got a survey concerning my call and gave him excellent marks. Received a Sam's Club eGift card for $45 a few minutes later from Sam's Club Member Services.
Totally unexpected.
On another note, chatted with AMEX a week ago and got nowhere. Got the standard 8-12 weeks pushback.
Sam's Club support > AMEX support. Who'd have thought it.
