No. You just need to be logged into a Google account that has never had Fi service before. Or at least 180 days since the last service I think.

Haven't received phone yet so haven't done it yet, but there's an account number in the boost "dashboard", your boost account page. Would make sense for that to be it. Or you could ask boost cs since you'd be calling them anyway to ask for the 6 digit port out pin (it's not the pin you set yourself).

As long as the 2nd phone has a separate email, yes.

Has anyone had issues getting into this deal after initial transaction/sign up getting botched? My bank decided me charging a phone on google fi to my credit card was fraud and declined the transaction....I decided to cancel the order right away and start from scratch to avoid any problems, but it seems I got ahead of myself. Looks like they created a fi account for me even though I cancelled the order, and now the promos show as pending in my "account", which hasn't been obviously activated...After back and fourth with support on phone, email and chat so they can clear this account from their system in a way that won't disqualify me for the current new customer promos, they insist they can't close it that I have to do it myself (which there is no option for because account hasn't been activated), and they now want me to create a new gmail account to use the promo.....B.S

I cancelled my Fi backup line about a month ago that is linked to my main google account. Could I sign up on this deal with my 2nd google account?

And I guess this phone is also unlocked, correct?

Why are you opposed to creating a new google account? In fact that's what you should do in the first place. Fi has a reputation of messing things up, and if you get in trouble with them it could result in widespread consequences to the associated Google account, so it makes sense not to use your primary Google account (or even secondary).

As for closing the Fi account, saw a past post on Reddit where customer service told them the account will automatically close after a certain time if you don't activate a line on it. If there's no service to charge. Once that happens presumably the associated Google account will be able to get a new a Fi account. But again, why bother waiting when you can just create a new google account.

I suppose you are right about creating a new email account just for this offer.....I just cringe at their obvious incompetence. I always end up getting burned somehow one way or another, no matter how hard I try to avoid these types of headaches...In retrospect, I'm still iffy on trying again with a new email since I will be porting in the same phone numbers tied to the initial attempt with my primary gmail account 😑

It's been said many times here that the phone number you enter on sign up doesn't mean anything. You can use a different number when doing the actual port after receiving the phone. And even if you use the same phone number, it doesn't matter as long as that number is not a currently or recently active Fi number.
