Welp, it finally happened after a year of it on my keychain. BUT it only happened with a CERTAIN kind of pocket. I was wearing gym attire that had MESH lining for pockets. So the nub part on the blade for your finger got caught caught in the lining. No injury but just warning it'll happen with meshy pockets!
