This is the second free sample sign up that will not let me submit because it says my last name is a required field. OK so why is my last name not good enough . This is anti-Italian prejudice at work I guess

Misspell your name intentionally.
That way you can track to see who they sold your name to when you start getting junk mail with the wrong spelling.

It did the same for me! I went back and checked it out since my phone auto filled everything.. and there was a space added behind my last name. I just backspaced that and submitted and it worked!
I used this and it works pretty well. It cost $35 at Costco for 2-5.3oz and 1-1.7oz tubes if anybody is interested in getting more after the free trial. A single tube at Walmart is around $24. Saved the smaller 1.7oz for travel.
Thanks OP! Used this stuff since it came out as prescription-only.
Best deal is that Amazon has just released a generic voltaren as an "Amazon Essentials" Diclofenac .
Also good to know -geezer alert - is that many Medi-vantage plans offer points toward over-the-counter meds such as this. For example, Humana Gold allows me the free points for 3 tubes of generic every 3 months. Check your plan! :-)

Ha ha I use fake name identified with certain free things so I know who they are selling my information to so I know who they are selling my information to
I have arthritis and have been using Voltaren since it was still prescription only. It really helps, much better than menthol pain creams.
That said, my rheumatologist warned me to NOT pet my cats after applying it unless I washed my hands thoroughly, or let them lick anywhere I applied it. It is apparently poisonous if they ingest too much of it, even from licking their own fur.
