tldr: Needs to be a ported in number. GV works (create a new gmail account, pay google $3 to port out). People who dont care about the 12 months mint service can just discard the activation kit and treat this as a $289 unlocked phone.

You might have some chance of getting support to let you upgrade the 12 months for the 6 month price if you do it right at the beginning but usually it is a no go.
You have 45 days to activate the SIM in any phone, as long as you port from another carrier (including google voice). Use referral program to get an extra $15 renewal credit.
My bank has been charged and is no longer pending but I have not received notification of shipment yet.... only the e-mail saying 'we are double checking everything and will send you another e-mail soon'
