Its possible to use international eSIM services like Airalo, which I've found to be inexpensive and reliable for international travel.
Depending on your needs, you can use Airalo in the 2nd eSIM slot of your iPhone. Then If you set your iPhone to only use data from that 2nd eSIM, you can get incoming calls on your primary US number, using the data stream from the 2nd eSIM for the call.
This setup will actually be WiFi calling, not airtime calling. On some US phone plans, this won't trigger international call charges.
Again, I don't know your exact situation, but there could be some possibilities for you with this route. Anyhow, I hope this helps someone out there who only has a dual eSIM phone…
Again, just trying to help…
EDIT:
If you want to know exactly how to do this, let me know and I'll post it a bit later once I'm at my computer where I have that exact "how to" info saved…
2 - Turn OFF data roaming on SIM1
3 - Set SIM1 as your primary voice
4 - Set SIM2 as your primary data source
5 - Disable the cellular switching
Whenever not on wifi, this setup will piggy back off SIM2 cellular connection as WiFi calling. This also works on iPhone 13 Pro Max using physical / eSIMs
Apple Support mentions DUAL SIM usage here:

Scroll down to "Learn about Dual SIM status icons"
I hope this help!

Just the US model is esim only.
https://www.airalo.com/spain-esim
For example, in Poland, the popular service provider "Orange" has an app that you can download and for a few bucks, you can buy a prepaid esim plan and get a local number and data within a few minutes.
The first time I used it I even found a promo code that let me get my first month on any "plan" for only 1 PLN, which is like 20 cents USD. I just selected the plan, checked out via PayPal, and a few seconds later the esim was installed and ready to go. Shame it's not like that everywhere tbh.
In Spain, one option you would have is also via Orange, but you would first need to have a physical Orange sim, then request an esim and pay a one time €5 fee. The only catch is it has to be done in store.
You could probably take care of that in one shot if you went to an Orange shop, asked for the €15 prepaid rate, then asked to have the physical sim swapped for an esim. Their website says this process works for any customer, including prepaid.
Since you're only using it for 2 months out of the year, I'd imagine you'd probably lose the number after some period of inactivity and you'd have to repeat the process every time you go there.
For now, I'm still holding on to my 12 pro plus an S22 Ultra, which also works great as a dual sim phone. I travel around too much to lock myself into esim only until it becomes more accessible.
I did actually try Orange last summer, but out of the 8 or so stores I tried in Madrid, Barcelona and smaller towns, NONE had any SIM cards available!
" I was not able to transfer my number to this phone on Sprint-T-Mobile network. Called Tmobile customer service and they said this phone cannot be activated on Sprint -Tmobile."

Thanks, appreciate you taking the time to try to help. I had looked into Airalo, but for the amount of time I spend there I'd have to do 3 x their 30 day option. The sim I use in Spain, Movistar, gives me 50GB of data every 30 days with calls and texts for €10/month, so I guess I'm spoilt on that pricing!
Yeah, most people won't care though. Too bad my FaceID is broken, otherwise I would just keep this 13.
You're right - they do. It's only for 15 days though I think, although I can't actually find any details about it now, so I'm wondering if they've removed it. But either way it was only for a short spell 14/15 days. I go for about 65 days, it's just not economical for that long vs a local sim option (Movistar €10 50gb calls/texts 30 days)
Yeah - it's an option, but would wind up being $78 for the time I'm there with much less data than I get via local sim on Movistar.
I'm on TMob too, fine if you're on vacation but I have to work and need lots of fast data.
See above. Expensive, was only for 15 days or so at a time, and now seems to have scrubbed all mentions online anyway.
Or use another phone that supports physical sim?

FWIW, in Poland, before I found out about the Orange app, I went to several of their stores and they also had no sim cards available. I think they must work on commissions because I speak the language, and nobody told me about the app lol I only found out after searching reddit and it was crazy how easy it was to get the service going.
The best options for data and cell service will always be local. As you said, the current esim only options are really just for short stays and they will always be a lot more expensive.
