How much time did they take since receiving your phone to accept the trade in? Will they apply the monthly credit retroactively?

I think they would as long as it postmarked and received by the date they give you for return. it took them at least a week or more after arriving to get to appraising it.

Yes, they use eSIM. Before the 14 came out they would use physical sims in phones that had eSIM but since then even 13s come with eSIM from them. The new phone will have the ported in number. Your plan is unlikely to work.
I got a 13 mini two months ago and it surprisingly came with the physical sim. Not sure why that is and I fully expected the esim. I was relieved and happy it came with the physical sim. It was an upgrade of an existing phone line if that matters.
