Ordered via phone with Costco. Like they said. Just T-Mobile team that is familiar with costco deal. Ordered 2. Traded an iPhone 5 and an old Moto e. Cost me $86. Supposedly will get gift cards within 15 weeks via regular mail. $75x2. Took over an hour. But 45 minutes was on hold to get to rep. Thought resale would be over $300. But it seams amazon is seeking for $250.

tmobile.com -> login -> top right corner -> contact & support -> check order status -> trade-in device -> print shipping label.

Some accounts might have issues with above.
Then you can also call tmobile support and they will email the label to you.

Can I have the the Costco phone number whoever processed your order? I went to my local Costco with T-Mobile yesterday, they not familiar with deal, I have to educated them what's the promo code, my phones not able turn on, I need let them check promo term, it doesn't require conditions, and finally to place order, system have the problem can't finish place order, sounds like most Costco can place order over the phone, I want to try your Costco.

The costco t mobile rep screwed up my deal. This guy messed up my imei numbers, not one, both were wrong. The only thing he really worked hard was push me to buy the insurance for these outdated phones. I have to keep telling him "no, thank you." For at least 10 mins he tried to tell me how important to have insurance, even i told him these are only for back up. Now I think he possibly entered the wrong imei in purpose. Please make sure you verify your imei number in your record, or you may end up paying the phone in full price. I have to refuse the shipment when it arrives.
