Where to buy prepaid debit cards with no fees?

I've seen deals pop up occasionally where there is no additional surcharge to purchase the card (i.e., a $50 AmEx gift card costs you $50).
But are there any b&m stores that sell AmEx/Visa/etc. gift/debit cards with no fees all the time? I doubt it.
If not, does anyone know of any deals going on right now for no fee cards? Thanks!
Edit: the card must be reloadable at B&M stores of some type with low or no reload fees.
