I never like Xfinity due to their billing and pricing once 12 month period expires. They tried to sell me Mobile services twice and I just flatly refused it. Very hard to deal with their customer service.

Phone order: Wednesday afternoon, Verizon kiosk in Costco
Phone arrive: Friday late afternoon via FedEx (about 48 hours total)
Port in (Saturday), new number to Verizon: This step required chat and then escalate to push port through. I recommend porting during "office hours" and to have another phone ready to call in as you cannot call on the port line. I suspect the port in was mixed up since I had to cancel a self order on the Verizon app to reorder at Costco to take advantage of the $250 gift card. This was exacerbated since the new 15 self activated with a Verizon phone number upon power on. There was certainly confusion/repeating in the chat (use copy/paste) to establish that the new number given to the 15 was going to be ported in/overwritten - and not port in another line in addition to the new 15. Port in was also my last step as I had already powered on, backed up 11, restored on 15, installed a glass screen protector and magsafe case. Fingers were crossed for sure. Total time: 3 hours, on hold and waiting for chat to escalate was probably 2 out of 3 hours.
After port in, I confirmed number was active and showing on Verizon account. I then created an appointment to drop off the trade in person. I dropped the trade in person at Verizon and asked for trade in receipt. Receipt confirms model, IMEI, credit ($10 strikethrough, $830, $23.06 x 36 months), date, time, location. In and out: 15 minutes including the parking garage.
Now to scan in the receipts and wait for the Costco card. Be sure to compare bills to make sure the discounts/plans match what was estimated earlier.
I'll repeat all of this in 3 years as I'll port out a Verizon number soon to go back to 5 lines (started with 5, port in non Verizon, port out back to 5). I'll swap numbers in 3 years to look new to Verizon again. This is how I purchased my 11 from Verizon. I keep the port out on red pocket for $30 a year. I plan to use both esim slots on the 15 so I can answer both lines with one phone.

Here's the math for my port in:
Pro max, 256gb, $1200/36 = $33.33
Unlimited plus plan (4 existing lines, 1 new unlimited plus) = $42
iPhone 7 plus trade in credit = $830/36 = $23.05
Costco $250 gc mailed or emailed after a few months. Verizon kiosk in costco printed a receipt with "$250 gc" on it.
Taxes are due upon upgrade. $35 activation was added to monthly bill.
Trade in will be dropped off at Verizon with submission ID when I receive the new one. Old phone needed to be on/online to confirm find my iPhone was logged out to generate submission ID. I recommend "trading in" at the same time (even though the device doesn't get dropped off at Costco) to ensure the credit is applied to the order. The non-promo trade in value is $10 so that'd be a $820 gamble.
Why am I porting in? Family plan is unlikely to change for 3 years. Paying the same monthly without a phone credit did not seem worth it. Verizon unlocks after 60 days so it'll be useful for international esim eventually.
Trade in credit varies with plan: $1000 for ultimate unlimited ($52/mo), $830 for unlimited plus ($42), $415 for unlimited welcome ($27). It is $3.5 a month more for unlimited plus from unlimited welcome since you get $415 more trade but you're paying $15 more for the plan.
Uprgades get $100 from costco instead of $250. Upgrades, however, require iphone 8/se and above to get the same value while new line can take in basically any iPhone.
The my Verizon app can show you an exact value on trade in (and new monthly bill) after going to shop, adding in the new phone first, select monthly payments, and yes on trade in. Do not completely proceed as you will not get the Costco gc. It will save time, however, since you can tell the Costco Verizon agent exactly what plan to build and you'll already know what it should cost ahead of time.

2 lines are iphone15
1 line is iphone15 pro max white
1 line is S23 Ultra
For iphone15 using 3 iphone 5s as tradein. For S23 ultra using iphone12.
3 of the phones should arrive by end of week. But pro max in white is listed as Nov 17th. As per instruction tradein are due within 30 days of receiving the new phones. So not sure how to finish porting, should wait till Nov or port the first 3 lines first and send the tradein .
Email receipt did show Costco GC of $250 for each lines. Rep told me no activation fees but I did saw the fees on the receipt.
I used 3 iphone 5s for tradein for iphone15 and iphone 12 for S23 ultra to get $1000 credit on a unlimited ultimate plan. Also pro max I used unlimited ultimate plan.
Taxes and fees are a whoping $32 per month surprise coming from TMO where its included in the monthly.
I chose "iphone 11 pro max" and got up to $1000 in credits over 24 months offer.
The Costco rep originally did not do it and showed a $210 trade-in value for my wife's iPhone 12 and insisted the $830 was only for new plans. But, I looked on his screen and saw he didn't click on the "see available offers" (or something like that) and when we clicked on it, there were 4 offers available, including the $830 trade-in credit one. Once he applied that, it worked like a charm.
