this comes to the primary address of costco account or can this be shipped to other address?
Think link has the Costco T-Mobile number where you can do this upgrade through the phone and get the cash card. Just ensure you ask them to double check that you qualify.

Edit: I think Costco just needed phone to be unlocked from carrier. They said is okay if screen is locked since phone is broken/can't turn on.
