You watch the horrific images and tension builds inside of you as the protagonists' life is put in increasing danger. Inevitably, you end up feeling their dispair. But when the protagonist sudenly and unexpectedly emerges victorious, all that built up tension - plus your own negative feelings from a bad day or a bad week - are swept away with it and you're left with a bit of hope.
So a well done horror movie can be threaputic even though it looks like a torture porn piece shit.
