I'm not an HTML dev, but the payment form looks weird in Chrome DevTools to my untrained eye. Each input is in a separate form element, so they must be using JavaScript to bundle up the form data when you submit. Finally noticed an API error is logged in the dev console:
Even chatted online with Max support yesterday; they advised to try a different browser and even a different network. That made no difference (Chrome vs Safari vs Firefox). I also disabled the ad blocker, but tried to stack with the Capital One Shopping offer. Chase assured me this isn't a payment issue; if it was fraud detection I would have seen a text/email automatically.

It's some kind of back end issue; nothing we try should make a difference. The only thing I won't try is to not use Capital One Shopping (looks like triple cashback today).
Anyway, MaxHelp on X did reply to my DM and said they have escalated it. So we'll see if their devs can fix this soon. If not, I can certainly live without the subscription.

For this reason I canceled and didn't renew and typed a strongly worded reason when they asked why I was canceling. I hope they forward it to UX team cause it's ridiculous for the payment interface experience to be that shitty.
