Saw this offer on multiple chase cards. Language is a little unclear as to whether two payments with the first being small and the second being $50 would qualify or if you need 2x $50 payments, but here it is. I got it 3/6. Word is there might be a VZ version floating around but I have not seen it personally.

You guys waited one month to make two $50+ payments or just paid it two times immediately in same day/next day?
Same day, one after another, within minutes. Two different payments for same Verizon account
Same day , just make sure 50 and 50.01 or something like that to avoid duplicate flag. If not pay today and tomorrow for $50 each

Yes, it did reset for couple of cards after Oct 1st
If you are asking for Verizon or T-Mobile I am unsure as I made postpaid payments.
As far as Costco offer goes, if you buy Costco shop card (Costco gift card) for exact amount ot $65 (I did not try other amounts and want to be sure it's same rate as membership cost) I got $20 back
Never tried online, did it in store in the membership desk area
Update: AT&T also allows you to reverse the payment back to the CC you used to trigger the offer on it. So if you have the offer on more than 6 cards and your overpayment has exceeded $300 - you can reverse the payment on the CC and use another CC which has the offer on it. Chase does not claw-back the offer posted on your CC (unlike AMEX). Just a DP!
My at&t offers on all 6 cards were triggered by At&t Uverse internet payment. I overpaid the bill (it lets you overpay till $300). I got credit on 5 - waiting for the sixth (my last payment was done on oct 20)

Did you get the credit eventually? I also got this offer on my checking account. I made the 2nd payment on 10/15 but no cash deposit in my account yet and didn't receive any offer redeemed email from Chase
