I just made sure to get the phone delivered and activate. After that's done, next day order went through fine.

It could very well be your address. They use LexisNexis for fraud risk which checks address, name, and phone number. You can try using a relative's or friend's.

Each line costs $40 ($35 for 1 month of service + $5 of Visible plan) correct? So, if one were to purchase 4 devices (assuming only iPhone 12) on 1 plan it'd cost them $200?

Line 1 - $80 ($75 for plan + $5 visible number)
Line 2-4 - $120 ( $40 * 3 )

You can activate online before the phone arrives. There is no need to open the phone and activate it once more after it physically arrives.

what if the ups delivery fail for some reason? like theft or some other reason. You probably won't be able to claim I guess.
