Oh no. I am not eligible for an extension (dialed *611 too) because I used the extension to place the order in the first place, and the phone did not arrive in 3 days. Can anything be done?

See if you can open another Metro account and buy another phone. Once that phone arrives and you turn it on and activate it, switch the line to this phone and activate it.

As long as I activate the phone and pay 1st month, the phone will be unlocked after 365 days? no need to refill 2nd month?

Thanks. Finally, I was able to place a new order with data connection AND a new shipping address. I think the new shipping address did the trick as I did try data connection previously and get the same error.

also got a few questions about the identity that I needed to answer before completing the order.
