Due to repeated questions in the thread here are additional details.
The $300 Verizon GC offer is tied to purchasing a smartphone online. You are eligible to claim this offer for every order transaction regardless if you are placing an order for 1 phone/line or 10 phones/lines.
The $200 Verizon GC offer folks have been receiving via sales chat/phone is a switch promotion. I am not able to find the terms for this offer but in the past this has been an available offer which requires you to port in from a non Verizon carrier that includes the various Verizon prepaids and MNVOs.
To my knowledge these Verizon GC offers have 12 month service terms or you will be charged back "claw back"
There are currently 2 separate iPhone 16 offers.

When you add the phone to the cart you must choose the right offer otherwise you will not go through the workflow with the step to add a trade in.
In addition if you enter a non Apple, Google, or Samsung device during the trade in you will receive the actual value of the device, including $0 if its older to be recycled,
AND you'll be defaulted to the non trade in promotion.
This is the reason at checkout some people are seeing $10/month or $5/month because they've selected the Welcome or Plus plan respectively and the non trade in offer has been applied.
There have also been a lot of "can I trade in XXX phone?".
The terms state you can trade in ANY Apple, Google, or Samsung phone in any condition. In my experience the any condition is intentionally meant to covered cracked screens and aesthetic damages. The device should turn on and function well enough that the IMEI can be retrieved and have no swelling/damaged battery (for safe handling reasons). Regarding any phone from the 3 manufacturers the process is constrained by the system and the list of devices/imei in its database. Be reasonable and don't expect extremely obscure or foreign region only devices to come up.
You can determine for yourself if a device qualifies for the $1000 by going through the online order process. You will need to enter the IMEI so you must have the device in hand. If you are not getting the $1000 value then you likely have not selected the right offer when adding the phone to the cart or your device is too obscure. I've attached screenshots of 2 devices that have been inquired about (A03 and 5c) showing the $1000 trade in amount.

I signed up for Unlimited Welcome, and also upgraded 1 phone to 16 pro 256 and another phone to 16 pro Max, which resulted in less than $8 extra per month. So I was quoted $190.38 before autopay discount and $150.38 after I activate autopay. This is for 4 lines on unlimited welcome in CT.

Update: Was finally able to submit rebates, was some problem verifying my email had to contact Verizon support to fix that, once that was fixed again to do the rebates.

Enter the promo code as stated earlier with your purchase date. You will get an email to confirm your email do that and it will then ask you to enter the mobile numbers you ported and give tracking numbers for each.
You may need to wait a few min after the first step before confirming the email as the first couple of times I did it, it just brought me back to the rebate page.

All this feels fishy to me, why advertise 16 pro as free but only apply 50% of value as credit? has the store rep f**ked it up? What are my options (online customer rep asks me to hang tight for 2 billing cycles, by then return window is closed!)
Was with Tmobile all this while and costco reps would do this very well and havnt faced these kind of incompetencies before. so kinda jittery at this moment about this whole deal.
on Top of it, dont have any notifications for the switch. $100 per line was promised for the swtich.
