I just received an email from T-Mobile from the Data-breach Settlement
related issue, and they have sent me an eMail with a Pre-Paid Card in the amount
of $56.54

The eMail contains information needed to activate your virtual Prepaid Card.
I took the easiest route and activated it, and used Amazon Reload and put in
the full amount and bought myself that full amount, so I never have to worry about
not spending the last cent on the prepaid card.

The eMail will come from :
and the subject is :

"our T-Mobile Data Breach Settlement Prepaid Mastercard Is Ready"

The bulk of eMail text says:
=====================
You have received a Virtual Prepaid Mastercard® worth $56.54 as your T-Mobile Data Breach Settlement payment. Please follow the directions below to redeem your payment. You can spend these funds online anywhere Mastercard is accepted or add to a digital wallet and use it with Apple Pay, Google Pay, or Samsung Pay.

If you have any remaining balance after 24 months, you may request that a new card with the remaining balance be re-issued to you. If you use your original or reactivated prepaid card at least once every twelve months, you will avoid any inactivity fees. If your card is inactive for twelve consecutive months, a $0.95 fee will be subtracted from your balance for each month thereafter of continued inactivity.

To activate and use your card, please click the REDEEM VIRTUAL CARD button below. After you redeem your card, you may use your card online wherever Mastercard is accepted, add it to a digital wallet and use it with Apple Pay, Google Pay, or Samsung Pay (or in stores that accept mobile wallet Debit Mastercard), or follow the instructions on the Card Details page at MyPrepaidCenter (using the website link below) for assistance with alternative payment options.
=====================

I won't include the redeeming instructions, as that is specific to your eMail.

Check your eMail box if you signed up to the settlement. It was a nice surprise for me.

End of email it says:

You received this email from T-Mobile Data Breach Settlement, PO Box 225391, New York, NY 10150

I didn't even know to sign up. It's funny how you have to sign up to be included, but you forefit your rights anyway. Not that it's huge, but I feel if you were affected, you should be included unless you opt out

Does anyone remember if there was a check option or just prepaid card?

I got the notification earlier today. @Fancy_Deer there was no option for a check. I ended up using the prepaid card to buy an Amazon gift card in the same amount which worked.

I got a letter in the mail from T-Mobile that my claim was approved and "You have been awarded a Settlement payment of more than $600. To satisfy IRS tax reporting obligations related to the payment, you must complete the enclosed W-9 Form and mail it to the address below, no later than July 7, 2025."

Cool that I'm getting more than $600. Not cool that I therefore will need to pay taxes on whatever I get.

I would hypothesize the 10 lines I had at the time.
