I purchased the Amazon SKU of the 8/256 last week, and on the box it has an Amazon specific bundle SKU: TB336FU ACC 8G+256LG-US-FO+P-WOA-AMZ.
Assuming the FO+P translates to Folio+Pen, and AMZ is Amazon.
Also, the "Included in the box" notes the pen and folio case for the Amazon listing, and not on the Lenovo site, but the wording is identical, so I would assume it's straight from Lenovo.
Is there some fee I'm missing?
Or is dead?

Not to be one upped by the useless eBay team, Lenovo's main 800 number and their email customer service was equally as unhelpful. Paypal, nor eBay would assist either.
TLDR; Don't expect a pen and perhaps be pleasantly surprised if one does show up
