Any idea if this is worth it? Thanks, also assume I wait till engine close to empty and put it in the fill on top
If you have a clogged injector, sure. I usually dump one in about 1-2 tanks before an oil change. If anything gets kicked into the fuel that combusts, it'll get caught in the oil and filter about to be replaced, instead of sitting there for another few thousands miles. Can't hurt the car.

Thanks, follow up questions. Is doing this the same as this service from the dealer (EFI SERVICE (FUEL INJECTION CLEANING SERVICE)). I just got the oil changed and they offered this at service time, so do you recommend holding off?

Just contains double PEA compares to premium gasoline which means pretty low concentration for fuel system failures. Pick Red Line sl-1 Techron Concentrated complete fuel system cleaner which contain much higher concentrated PEA to clean built up and clogs.
That's not how engines work at all. No need to do this before an oil change
I never said need. Best not to infer things people didn't actually say to make an argument.
It is a good time to do it regularly, without following the ridiculous schedules some of these products advertise, and helps ensure the cleanest oil.
It is possible for contaminants stuck in the injector to break off into the engine. That's where stuff normally goes, because fuel, you know, goes into the engine.

This is a very interesting video that answers many of the questions you should be asking before buying any fuel additive.
