The survey will be available from 9:00 AM Central Time on July 30, 2012 until 11:59 PM Central Time on October 15, 2012.
To be eligible, all entrants must meet the following requirements: 1) complete all questions of the online survey; 2) abide fully with these Official Rules, 3) be at least eighteen (18) years of age; and 4) be a resident of one of the 50 United States.

One entry allowed per person.
PRIZE: The prize shall consist of an Apple iPad 2 valued around $599.
