Also, since activating online worked perfectly, and phoning in was a disaster, does anyone know if it is possible to just create a separate account in my wife's name and do the online activation again?

Many people have, depending on the model number and part number.
You can swap phone anytime. You just cannot upgrade (i.e., lease a new phone or buy a new phone with installments on the line) before 120 days.
See my comment above regarding the iP5s.
A few people have used Boost devices on this deal. I remember LiterallyUnlimited said he can't change the MVNO flag on Boost devices, so you probably has to ask Sprint.
ADDED: I remember you asked about the Moto E4. The one from amazon works on this deal.
1. Yes. Sprint owns Boost and can set its policies to keep people from using cheap Boost phones on Sprint.
2. Yes. This is basically an FED date that is contingent on meeting Boost's unlock policies (active for a continuous year, etc)
3. Yes, as long as it passes FED. There are some phones that pass FED but are marked as prepaid or MVNO phones, which Sprint makes ineligible for its network.
To clarify: Boost phones are, largely, unable to be activated on Sprint proper until they've met Boost's unlock policies. Even if you get someone to flip the flag (though my system won't let me), it will likely fail on the Sprint BYOD side because it's not met all the other requirements from Boost.
Anyone who's successfully gotten the switch flipped on a boost phone that passes fed check and used it to sign up for this deal let us know how you did it!

Yesterday I've called the number in the Wiki for the third time, they worked for 32 minutes doing something, told they have fixed everything. Checked today - the plan is still incorrect. Then received a call from another Sprint agent who listened to me for 30 minutes, taking notes and then told he will call me in 2 weeks to check if the problem resolved.
Pretty ridiculous...
