Yes, as soon as the imei number on the sprint site showed it was eligible, I called Sprint to add the line to the new account. As edog11 said, you add a 1 to your phone number as the t-mobile account number. The thought or porting and reporting is overwhelming, but it's worth it in my opinion. I moved 5 lines. The bulk of time you spend is listening to the rep explain terms and conditions...
I was in the "if I order a sim online, it might not get to me in time" boat. The digital delivery method eased my concern.

It's only "digital" because you get the ICCID (sim id) via message. You don't need to physically put the sim in your phone to port your number.

I signed up for this plan last year and it ends in September. I used my SSN and credit card to sign up, and my daughter has been using it with an iphone 6.

My daughter now just turned 18. She has no credit. Is there a way to cancel my account and have her sign up using her SSN? Or is her lack of credit going to be a deal-breaker? If she can sign up, can she use the same iphone?

She can try. I used my SSN to set up our family's initial account. This time around I used my wife's SSN. All phones and sim cards can be reused for Sprint.

Edit: all phones that work with Sprint can be reused.

I didn't use that digital card but when using physical card never inserted into phone.
I guess there could be some risk that a physical card maybe needed to verify your identity(through text message) if your port doesn't go through or you forget tmobile pin.

Do an online chat and tell them you want to do a "device swap" (don't call it an "upgrade") -- give them new phone MEID/IMEI and new SIM ICCID

Thanks, got the SIM from Sprint yesterday and got the part number for my model from a link in this forum from Hunter and from a kind SDer's pm. But you're saying another place to get one is from an Apple store, but only for iPhones though, right?

I'd probably look through the Wiki and read a few of the recent pages of this discussion to get a better idea of what you need and how to go about it. Time is getting fairly short, but IF you've got Sprint compatible phones and can get Sprint SIMs for them locally, and are in an area with good Sprint coverage, it's definitely worth pursuing.
Also, the web link is sprint.com/1yearfree I would use that to check your phones and numbers for eligibility, but then still call in to take it further (preferably after you've rounded up all the necessary info and gotten ahold of the right SIMs, etc).
