I looked at it and it seems to me that it does.
Plus they ask the date of the damage on the claim form.

Well, maybe I got lucky. As I said, I just took it into the store and told them the problem with the mic. They took it, ordered the necessary parts, fixed it and handed it back to me a few days later.

Is the next date for this plan extension some time in March if they do extend? Thanks. My plan will be up 04/30, should I do it now or wait another month? I know this is a dumb question.

Yes they will have to do something some time in March or on April 1 - either extend again or not. Otherwise new subscribers would start getting less than the advertised full year.

I myself am waiting until at least early March to decide.

IF they extend again, right after that would be an ideal time. If they cancel instead, I am thinking they may give at least a few days' notice or there may be signs but that is NOT guaranteed.

Can someone help me with a couple questions? I read the instructions in the beginning and something doesn't make sense. It says to call in to get the deal and they will waive the sim card shipping fee. It also says to have your Sim card ICCID# ready. So do you call in to get the sim card shipped and then call back with your ICCID# or is this step only for people who already have Sprint Sim Cards? Also, when I call do they port over my number right then? I am with T-mobile and have never ported my phone number before. Does this cancel my t-mobile plan automatically? How long does it this take? Will I be without service for a while?

The Sprint version and unlocked(unbranded) version should work. The Sprint version should come with sim 416TQ and unlocked version you will need 516TQ or 416TQ. My guess is to expect activation problems on first few days after launch.
