Most of the questions asked here are either in the promotion posting, or are just stupid. Read before posting.
The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.
How much does ___ cost at Costco? Easy - go to the website. Prices are listed.
iPhone XS through Apple gets you $500
iPhone 8 through this Costco deal gets you $580

If you want monthly credits, tmobile is doing a deal as well with monthly credits.
Looks like they aren't that stringent when they get the devices.
finding one in stock is the biggest issue
I remember one time I went at about 6:30, about 2 people ahead of me. Customers were doing at 7 and the kiosk reps were telling them to come the next day.
Reason being, these add a lines can take anywhere from 30 minutes to an hour, maybe even more because customers usually don't back up their information at home and are at the kiosk uploading their stuff
We currently have two line, so if I add a new line and trade in a 6s plus; We get a small bill credit and the rest on a virtual Visa card. Do I need to pay for the phone outright when i get it or do I have to finance it though their company or is it just billed monthly on my existing t mobile bill? I understand the rebate offsetting the cost of the phone but I don't know how the phone is paid for in the first place.
Let's use the iPhone 11 64gb as an example.
The price is $629 PLUS tax at Costco.
You pay this Up front, at the Costco register.
Let's say Costco trade in $ for the 6s Plus is $30. You get the $30 as a ONE time bill credit on your tmobile bill. You would get the remainder $550 as a prepaid card.
IF they don't have the phone in stock at Costco and they have to order it for you, you don't get ANY type of discount, so now the price for the 11 64gb is $700 and you'd get monthly bill credits.
Finding a store that actually has them in stock is the hardest part.

It CAN bet network locked. It cannot be iCloud locked.
Please keep in mind with boost pre owned phones that the condition can be good or bad. It's hit or miss. I ordered a 7 and it came with a faulty charger port. 3 other iPhone 7's came in ok condition.
The same thing happened to me.
They initiate the process for adding the line and then rebate and eventually ask for the IMEI# and then print the rebate instructions paper and also the prepaid shipping label which has these instructions (screenshot attached).
Of course they quickly do a visual check for phone condition (mainly cracked screen) before taking the IMEI# but not the actual phone.
Once you complete the online process after coming home, you ship that phone using the shipping label they provided
The only pain-point I saw in my case was, since I am adding a line in my friend's account, I need HIM to log into his T-Mo a/c and submit the application (which needs a photo of Costco receipt and IMEI sticker on the iPhone-11 box) so basically I need to take my receipt and box to him to help him submitting the online application.
That's it as far as I can tell !!
And yes, I specifically asked the T-Mo Cust Care if the new phone device and SIM are attached for this PROMO (meaning do they HAVE to be together and they said "Naah, as long as the line is active, you should be good") because you are free to use any device you want once you start the line (which is always the case)!
They are either lying or clueless !
Costco never charges any SIM or activation fees, and also they are at least $50 less than market/APPLE's official prices!
can someone let me know? Someone in this thread said it was posted in a past deal but i couldnt find it

Others in the past have used it in Venmo to send money to a trusted other friend/family member - or to a 2nd Venmo account they already had.
As a homeowner I have enough regular bills I can zero a card easily.
