Most of the questions asked here are either in the promotion posting, or are just stupid. Read before posting.
The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

Can I port out my Google Voice and avail this offer? Or do I need to get some other service before I port it out?

Not sure what to think of your statement given that Apple says the opposite. That and I was able to park the new nano SIM from my XS last year in an old iphone 5s just fine after putting the nano SIM from my iphone 7 into my new XS since I was trading in the 7? Are you suggesting the 6s had a different SIM than the iphone models before it and after it? That seems a bit odd, no?

I may be unclear on your ask here -- you should be able to pretty simply cancel the extra TMO line after you get your rebate. I seem to remember doing so from the website with no trouble, but I could be misremembering.
Otherwise if you want to cancel the extra line by porting it out, NumberBarn is an inexpensive service to do that with. Then you cancel the line at NumberBard pretty simply.
No.
If you cancel a line, you have to REACTIVATE AND add a new line in order to get this promo.

1: your copy
2: copy that gets sent along with the trade in
3: shipping label
parts 1 and 2 have the imei number of your phone and the trade details.

You can simply open up a new line (no port needed)
If you really want to port, yes, google voice is cheap, $3
I think what you described is the exact reason why they put 90 days limit there. They want to attract real new customers, no old customers who temporarily cancel an existing line and then re-appear it as a new line to qualify the promotion. So if you have cancelled a line in the past 90 days with the expectation of jumping on a big deal during the holiday season, they ask you to restore that line first before talking about a new line; otherwise, they won't regard you are really adding a "new line".
The 6s and xs max SIM cards on the same size.
I have an Xr and the SIM card tray is 'backwards'. Maybe that's where the confusion is.
That said - depending on the plan you're on, adding a line for a couple months then cancelling it still leaves you in a pretty good position.
Though I think the 6 is too old to trade in on the current promo.
Sell your iPhone 6 on Craigs and use that money to buy the 11.
on my ship label papers, one of my iphones are listed as the wrong memory size. all the info of IMEI is correct, but it says 16GB and should be 64GB.
is this something to worry of, or not a issue since the IMEI and everything else is correct?
