I can't comment on the mess up part, but for the online submission step, actually everyone use the same code. Check the offer detail page I posted in the Wiki (the 2nd and 3rd page). It contains the promo code, which should be costco2019AppleTrade. Your line is what is really unique and qualify you the promotion.

BTW, you should also receive the rebate form (three boxes on that paper and they are customer copy, receipt, and prepaid shipping lable). It will be problematic if you don't have that.

Thank you for the reply. No I did not get the rebate form. I did initial and sign a document, but I don't remember which one. Would you be able to send a pic of what the rebate form and receipt look like? Thanks a lot

First of all, nobody the iPhone 11 will be free after this. You expect too much if you think so. It is just at a cheaper net price than MSRP $700.

OK, I think you mean 4 lines, then your calculation should be:
1. Buy 150 online x4 = -$600
2. Trade it in and get 580 x 4 = +$2,320
3. 3 months x $140 Magenta Plan = -$420
4. pay off iPhone 11 in store 630 x4 = -$2520
Total = -600+2320-420-2520=$1220, or $305 per line.

It is still 43.5% of the MSRP. Just consider your self to buy a unlocked iPhone 11 at more than 50% off price.

Thank you! It makes sense. If i cancel how long can i get signup with my social security if there is another deal? 90 days? Lastly, is it worth upgrading to an iPhone 11 Pro Max or the iPhone 11?

OK, I don't think it should be called rebate form but a trade in shipping form. Here is a copy.

I just thought of a follow up question. Did you get the rebate form from Costco or T-Mobile? In other words, did you get the form after you entered the promotion code on T-Mobile? I entered the promo code, and it found my promotion (screenshot attached). Is this gonna get me the rebate form? I haven't clicked accept terms.
It also bothers me that the Costco rep didn't check my trade in phones, and that the rebate form has the info of the trade in phone on it. Thanks again for your time.

Update: now saw your attachment. No, that's not the trade in form you should get from Costco. What I posted at #1572 is.

There is no so called rebate from that I got from Costco. I did get a trade in form that I posted in wiki (last picture) per others' request. Take a look there or just look a few posts above.

The online submission only gives you a confirmation page including the tracking number. That is not a form.

Those are actually 2 different things. It can be locked, but it needs to be paid off. An example is a total wireless phone from best buy is bought and paid in full the moment you buy it. But its locked to total wireless for typically 1 year
