-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

Are you willing to go to Brooklyn? They show the regular 64Gb in red and white, and 1 of the 128Gb in white. Also the ProMax 64gb in space gray.
When they were offering the physical rebate card, I tried to double dip by using the card to buy another iPhone XR and it would not accept it.
1 of my trade-ins was a Boost 6s+ "This device has not reached financial eligibility.", so that may help with a few questions others have been asking.

It might be this store's manager decided not to sell anymore as people may cancel after 3months but he did not ask me because i am going to keep tmobile for long time and many months
Representative at Costco told me that the Virtual Mastercard could not be used at Costco and could not be used to pay Costco Citi card.....Does anyone know if this is true?

But on 23 point, you said 'At Costco, you will get a quote for trade-in value of your phone based on its specs. Once you mail your phone and Assurant receives and inspects it, T-Mobile will give you a bill credit in the amount of the quote (or they may adjust the quote if something about the phone is different, i.e. your iPhone is locked to a carrier instead of being unlocked). You will then receive a Mastercard debit card in the amount of $700 MINUS your bill credit (assuming that you get $700 rebate -- see the bullet point about the UP TO verbiage).

#11 is a statement that carrier locked iPhones can be traded in.
#23 is not saying that you can't trade in a locked phone, it's in reference to the trade-in value of your phone. T-Mobile assigns different values even for same phone size. So a Verizon iPhone 7 32GB may have a different trade-in value than an unlocked iPhone 7 32GB. If the Costco rep mistakenly put down that you are trading in an unlocked iPhone 7 32GB when in reality you traded in a Verizon iPhone 7 32GB, your one-time bill credit may actually be lowered. Or if the Costco rep accidentally put down that you have an iPhone 7 64GB, but you have a iPhone 7 32GB, then your one-time bill credit may be adjusted as well.
