Right, sometime the porting request times out and need to resubmit info.

Your redemption instruction will be sent to address A (the ATT service address) if it is sent via mail. The address B, which is your cc's billing address, will be the address used by your credit card issuer to send you the monthly CC statement. But Cc statement that is sent to address B will forward to your address A since you submitted a mail forward request to USPS. I believe ATT will only send monthly bill to the address of the primary line account holder. One of the 5 lines will be the primary line.

Must port in to qualify for $250, the goal of switcher promo is to take customers over from competitors. Good thing about this deal is it doesn't required being a NEW customer. So existing customers can participate

can't say if this issue is casued by exactly same problem but the similar incident happened to me when doing $299 iphone SE deal with Sprint.

They could not open my account because they said "cannot verify identity" and asked me to go to a store.
I went to store and had to call in to security team, but finally I got approved but I had to open a new line with Sprint over the phone because the dam.n BestBuy still couldn't open a new account under my name.

Anyways, I opened my first line with Sprint, then added the line at BB.
Just had to port out two lines after that.

So maybe if you go to store and verify ID, it might fix your issue. then add additional lines online as an existing account because it seems to be of least problematic option.

Opening the first line in-store and 3 more lines online (Share Plan is capped at 4 lines) increases out-of-pocket costs and reduces the profit (see attachment). I'll only do a deal if the profit will be equal to or more than the out-of-pocket costs.

As shown in my spreadsheet, the profit is $310 for an outlay of $490. Maybe I'm picky, but that's not worth the hassle FOR ME. Others people might not be as picky.

I resubmitted the order yesterday. If it goes through, then the profit is $560 for an outlay of $470. That's better.

I just talked to Ecom (I set a hold limit of one hour and they answered after 35 minutes). The rep asked for last 4 digits of SSN and billing address. She said she's made a notation in my order that she had talked with me and obtained the info. She said it's now "being reviewed" and couldn't say whether or not it'd be approved (or why the previous order was cancelled "Sometimes our system has problems with online orders".). She did check and confirmed that the $250 GC is per line, not per account.

Update: after reaching a rep at ecomm, they literally couldn't find my order in the system, which was pretty frustrating. Then my internet randomly flashed in and out, so the call got cut. At this point I nearly gave up and resolved to call later in the afternoon.

The crazy part is, it seems like my order went through. I got an email update that I'll be receiving a shipment confirmation soon. I guess it really is a hit or miss. Upon checking the porting status online, it shows confirmed and expected time of completion this evening for all 4 Boost lines I bought from ebai.

5/3: Used BF cb site and went through to ATT website, selected share plan and ordered 5 lines (that's the max I could do in that order online). All 5 numbers are coming from a valid/active T-Mobile account. I confirmed the ssn/Tmob acc/pin during the number validation and addition. Upon completing filling up details and validation for 5 line and moving to checkout I keyed in cc info and my ssn and billing address and submit to go through credit check. Upon completion it did allow me to submit my order. Got an email confirmation about order. But it did not show up on their site until midnight 5/3
5/4: saw an email at 6am est that they are working on the order and will update. At this point order is show up for tracking. 10 am I got a cc charge of $1is when they verified my card on the order and later I got an email that order is in progress.
