If you do it through T-Mobile there is a sim card/service fee attached to it.
First, I verified that my rate plan qualified.
Then went to Costco (when it just opened, so was first in line.) The guy was sort of new and wasn't familiar with the "add a line free" deal. No biggie - I just asked him to add a BYOD line anyways to take advantage of the $200 shop card. Probably took about 30 mins since he had to get his manager to help enter it in.
When I got back home, I called T-mobile CS and they were able to confirm this newly added line is free.
Weighing whether it's worth the trouble to try for the $100 port-in rebate. Anyways, at least the $200 was fairly easy.

I have 5 phones: 3 unlocked and in use and 2 new Samsung Galaxy A71 from T-mobile from one of the last Samsung.com deals.
1. How can I get $200 Costco GC's (5 of them or maybe more)?
2. How can I get $100 port-in deals from T-Mobile?
3. What plans from T-Mobile can I sign up for: paid lines (the less price the better) and free lines?
4. Should I get Costco membership before getting to Costco's cell phone rep?
5. Will I be able to make it all go through by tomorrow's night? (have to pay for the new Cricket cycle).

The rebate redemption form states that the Control number cannot be duplicated.
The promo code is same for all, but the control number is unique.
The reps should be adding a new line, BYOD IMEI, etc, and the system generates and prints the rebate redemption form. The rep gave the printed form which had all the information - promo number, control number, purchase date, IMEI number of BYOD device, activated permanent phone number, and Kiosk number. I came home and submitted the rebate after inputting all that information.

now just need to wait until Nov 2021 and figure out how to move existing line on top of the FREE lines. aka, port-out and port-back-in since now "2020 Line on us 2" and "2020 line on us 3" i'm certain were added at Line-level, not account-level
