the guys at the kiosk had some difficulty printing out the rebate form but then they got it done but i'm wondering if anyone has a different promo code than T01491?
I asked the rebate people and they referred me to wirelessadvocates (the company that runs the costco kiosk). i'm hoping i get faster help in the forums online.
Since the promo has been extended, if anyone has gotten a promo code in the last couple days, that should be the right one for me to file under, so any help is appreciated!
I'm hoping there is someone out there that purchased on 12/1-12/3 with the right promo code!

Unfortunately, your best bet would be to go to Costco and raise a ruckus.
the kiosk reps had a hard time printing out the form and then they went back a day and printed out the form but i got my own control number so it should work.

The $50 however reflects what your additional line charge would be without any discounts (for auto-pay, anything else). So, for example, on my Magenta plan, it said $25 and $50 but would actually be $20 and $40.
Total Monthly Charges $25.00
Total First Bill Charges $50.00

Another option would be to put the purchase date as 12/1. That's how they got the rebate to print - by going back a day. So you should be fine. I am not sure if they actually check when you added the line, they look at when the Control Number was created. If it does get rejected, you can then raise a ruckus.
