Porting in from various networks: don't use Google Voice, Boost or any MVNO that uses Tmobile as their network. Even if the rep let's you do it, your rebate will get rejected later.

It is not known if you can stack the costco $200 with the tmobile $100 but one thing to note is that the costco rebate is by Wireless Associates which runs the booths in Costco, while the $100 is directly by T-mobile.

I believe you can do that only once. If you have used ACH before, you cannot use that again. Unless you use a different bank account.

If you did it before you need to go back to that promo and delete your bank account info from that old promo. Then it will work. I have done multiple rebates transfer to bank without problems this way.
Somebody on Slickdeals or Reddit wrote about it. I just happen to catch it while searching for the solution.

So my rebate was denied again after resubmitted. Chat with T-Force and was told because I added a line at Costco. Anyone here was denied for that same reason?

ACH = transfer to your bank. Once you log in, click on menu on the left, then select Access Funds>Bank Transfer.

Just checked mine and was denied for the second time. First time they said it was because I used the primary account line as the number to contact and send the text to. Later, I resubmitted with the number for the line that was ported in and it still got denied.

I did add mine and ported in at Costco too but I don't think that should be the reason for denial.
Will have to contact T-Mobile about this and have them fix it on the spot, I am not waiting around another 4 weeks for this rebate.

This! Found the old SMS from 2 months ago and deleted the bank there and then the new one works just fine for ACH.

Same here. Got denied 2nd time. Checked with T-force, and they said it was due to activation and port-in done at Costco. While I was waiting for T-force to respond, chatted with TEX and they said the same thing. I mentioned to them that there was nothing mentioned about Costco in the promotion, and they applied $100 to my account. The rep from TEX did warn me about Costco for future promotions. Most likely since Costco runs its own promotions in conjunction with T-mobile.
