If you bought the original game - via disc or from the PS digital store, you get the PS5 upgrade free. If you're only getting FF7R via PS+, you DON'T get the free PS5 upgrade since you didn't "buy" the actual game (you're only leasing it while you have an active PS+ account).
The Yuffie DLC is an extra expense in any scenario UNLESS you pre-purchase the NEW "Intergrade" pack which has it included (hence the new offering).
In short, if you bought the game originally, you get the equivalent upgrade to the PS5 version free and will pay for the new DLC like everyone else. If you got the game via PS+ in March, you don't own the game and thus don't qualify for the free upgrade.
This isn't rocket science. Did you actually purchase the game? Free upgrade. Did you get it via subscription? You didn't purchase the game meaning no free upgrade.

If you own the PS4 DISC version and have a PS5 DIGITAL (no disc) system, you don't get the free PS5 upgrade as you have a DISC license, not a digital one and since your new system can't play discs - well ... if you purchases the digital version on the PS4 and have the digital PS5, you're fine since the license remains the same across the platforms. same with disc to disc. Disc to digital? Again, that's a negative.
Also to clarify, the Yuffie DLC is PS5 ONLY - meaning it doesn't matter which version of the PS4 you have, unless you have a PS5 sitting around somewhere, you aren't playing the Yuffie DLC - it won't be available to you at all.
