Only the Platinum card. Also this is in-house cell phone insurance by Amex, Visa and Mastercard's cell phone insurance is administered 3rd party and they are a pain to deal with
To get coverage:

You are eligible for coverage the first day of the calendar month following the payment of your Eligible Cellular Wireless Telephone bill using your Eligible Card Account. If you pay an Eligible Cellular Wireless Telephone bill with your Eligible Card Account and fail to pay a subsequent bill using your Eligible Card Account in a particular month, your coverage period changes as follows:
Seems to be AIG subsidiary, "Coverage is provided by New Hampshire Insurance Company, an AIG Company."

Fyi, rented and leased phones are not covered under this AMEX policy.
Checked my Mastercard world elite policy and that specifically includes these phones.
When you say leased does it mean if we finance the fones like monthly installment or what att or tmo called EIP then you cannot? So only outright purchases at full retail?
I stopped using my Amex card as frequently after switching to the Chase Flex CC, which already offered this. Smart for Amex to offer this to compete. I bet they've taken a hit to Chase offering this ahead of them.

This is their exact language for phones that are NOT covered:
"• Eligible Cellular Wireless Telephones which have been rented, leased, borrowed or Cellular Wireless Telephones that are
received as part of a pre-paid plan"
I've used the freedom flex (free card) coverage and it's pretty easy. They just need a receipt showing the repair quote. Ubreakifix has no issues getting you a free estimate on the spot. As long as you paid the PRIOR month's bill with your card, you're good for the coverage.
They don't need proof of where you got the phone or what you used to pay for it. I straight up asked the shop to replace *everything* in my note 9. Screen, battery, charging port, back cover, and stylus. Came out to about $480 (LOL). Only paid $50 deductible.
Amex warranty services have become MUCH less user friendly since 2020, when all their free cards dropped the coverage. Even calling their phone number you get put through more hoops than back in the day.
What documents did the need ? Other than the estimate and the screenshot of your phone bill with the phone being listed. I'm going to get my phone screen fixed and have been paying my T mobile bill with chase freedom flex. Thanks
Something to consider is that some carriers are shifting towards new AutoPay/eBilling discount restrictions. Verizon doesn't give an AutoPay discount (which can be $10 per line per month on some plans) if paid with a credit card. May not be worth it depending on how much insurance would've cost.
whoa. glad tmo doesn't do that. we get $10 discount and i use my chase ink preferred (which has cell insurance)... best of both worlds :-)
Can you buy a broken phone and use this claim to get free repairs? How will they know if you brought it broken? Do they need sales receipt of phone purchase as new?
Where do you bring it? There's no samsung store is there?
Yes there is, at least in my area in NJ. When my S8 stopped working, I brought it to them. They took a look and provided me a copy of estimated repairs which I submitted to file the claim.

They won't know. They've accounted for insurance fraud from scumbags like you. They just blacklist the ones with a history of it.
FYI-US Bank Kroger Mastercard started offering it last April. After 2 bill cycles I submitted a claim estimate for screen repair of $200 and they reimbursed $150 after $50 deductible, because I wanted to fix it myself.

You have to show proof that the phone is activated under the account using the credit card to pay for monthly bill.
