I believe the larger size is more about having more adhesive contact for less chance it loses grip. I *think* they started with only the original smaller size and later added the larger size for that reason but I'm not sure. I get the feeling they might be getting rid of the small/medium tan completely as most stores stopped carrying it and its been harder to find online too. But a lot of stores do carry the clear in small/medium and I'm guessing that will stay.
I used the small/medium for a few years before ever trying the large. I've been buying either since and am still undecided as to which I prefer.

I suspect if I tried them on right after each other to compare I'd find the clear must be slightly less effective due to the weaker adhesive. But it still worked fine for me and I liked the ease of removal.
I haven't used clear enough to know which I prefer. But I did order the max of two clear boxes with them being cheaper and then stocked up on tan as well. No idea regarding breathability/breakouts. The clear is more plastic like but if I recall correctly didn't feel like you were just slapping a piece of plastic on your nose. They were comfortable to wear.
Neilmed sinus rinse is even easier to use than a neti pot. It's like a squirt bottle that shoots the saline upwards. Google it.
That makes sense. The packaging for the clear states that they're better for sensitive skin, since that version's adhesive comes off easier than the tan version's. I guess the manufacturer discovered that the adhesive was weaker and tried to depict that as a "feature".

It helped my snoring and helped clear the sinus, nasal congestion issues.
But now I'm lazy after a year tired of those daily nasal irrigation remedies.... wishing for installing a sprinkler system 😂😂😂
