At my location, a medium pepperoni is $14. That's makes this "deal" irrational to not just buy the much bigger large 3 topping for $7.99.

But that's how upselling works. Their goal is to get as many dollars from you as possible. Yes, you get a slightly better price per unit pizza because you are buying a larger quantity, but that pretty much applies everywhere with everything you buy.
