$150 Costco cash card and $250 T-Mobile Visa gift card. Must be on Go5G plus plan for 90 days. They have a current promotion where you get a third line for free if you get 2 lines. Rep at Costco also waived activation fees.
I activated 4 lines at $190 per month (taxes included). I'll stick around for 4 months - $760. I'll get $1600 in rebates.
