Does this work if I had a old Quicksilver card but account has been closed for 2 years? I just tried my old CO login and was able to log in except no accounts are associated.

Doesn't look like it. One GE/TSA Pre per 4 or 5 years is a separate benefit than the $300 travel discount which you use from the CapOne portal from what I gather.

Yes, you should be able to apply for and get this card with SUB. I also had Quicksilver when I applied for this card and I got SUB.

Cap one is a bit odd when they do theirs. When I applied and got denied a few years back, they pulled from one. According to some research, it looks like they sometimes just do one bureau, but it is seemingly random as to which one it is. Sometimes they pull 2 or 3... and your location has influence on its frequency.
