The shop card offers at least from internal docs do require EIP/Device on monthly payment plans. Whether T-mobile follows the terms they made is another story.

Once again the image on the first link you posted is from January of this year, around March they published the phone number that appears here and I believe that's when they started making the whole process automatic without Costco members having to fill out any forms to receive their cards.

Why your local warehouse has that sign up?I have no idea considering Magenta plans don't even qualify for any of these offers anymore, they haven't for months and months, you need a Go5G Plus plan

The tmobile reps did point out that the Magenta references were outdated and it's been replaced with Go5G+ and non-Go5G+. I'll take your word that BYOD works since it's possible the terms has changed since Go5G+ was introduced.

I went to Costco today and I have a prepaid AT&T SIM. They said the 250 card is only valid if you switch a postpaid wireless plan, and 150 is if you buy a new phone, 75 only for BYOD.
They won't offer 250 cash card for my prepaid SIM

Stay away, they're using any non legitimate reason to deny rebates, even if you genuinely met all qualifications. After you give them a call to inquire about rebates the first thing they check is if you in fact ported in your number from one of the carriers posted in the fine print, after that they resubmit the promotion for your account and tell you to wait the same amount of weeks/days again.
They're obviously doing this in the hopes people forget about rebates or offer them a statement credit at best

I am on One plan and I worked with the T-mobile rep on Twitter and he transferred all my discounts (Autopay, insider, Line on us, kick-back) to Go5G+ plan which would cost me $120/mo for 3 lines, which would almost double my $64/mo One plan.
Is there any way to get better price for the 3 lines on Go5G+?
I would take it if it was under a $100 which seems to be reasonable for 3 lines.

When does it end ? I have more monthly payment remaining on Verizon ending on 1st week of Dec. Is there a end date to this offer please.

I haven't done the deal yet. I am still weighing in on my option. I hope he is correct and you get the gift cards. I was told by my Costco T-Mobile rep via text that my AT&T prepaid will qualify for the $250 rebate with the Go5G Plus plan on top of the $150 Costco card for each line up to 4. However that was before what CalmCalendar said. When I am ready I will be under the impression I will not receive the $250 per line as the worst case scenario as long as I get the $75/$150 Costco gift card and the full $830 trade in billing credits for 4 iPhone trade-ins.

I was in discussion with the guy at my Local Costco. Initially, he said that US Mobile ( which is a Pre-paid MVNO Wireless service which uses Verizon network) port-in to T-mobile will cover for $250 credit. We were not ready to do it on that day. When we went there next day, another lady was there while we were waiting for the guy to come so that he can close the deal for us. That lady checked the fine print and she confirmed US Mobile will be excluded for the credit.

Once that guy came back, he initially said that it will be fine with US Mobile. Luckily, we ended up not getting it no that day and postponded for the next week. At that time, he said that he will deep dive on the credit part and check whether US Mobile really gets covered or not. After few days when I checked with him over text message, I was told that US Mobile is considered Pre-paid and it will not get you the credit. ONLY the postpaid plans from Xfinity, verizon, AT&T and few others are covered and not the prepaid plans from any MVNO.
