Just crush a lysine tablet/pill with a spoon on a paper towel. Put the powder in a water bottle cap (Poland spring type) and slowly add a bit of water to make a paste. Works wonders for me after spending lots of money on snake oil.
I am a pharmacist and this product does work along with taking lysine a couple times a day orally. It is vital that you start treatment this second you feel that you have one coming on. I just went through a treatment myself earlier this week and my tube was dated July 2021 and work just fine for me. I do apply the cream probably six times a day for the first couple days.

For all the people wasting time on this stuff. I used to get sores 5+ times a year on my lips. Abreva helps but does not work nearly as well as what you can get from your doctor. Good to know that it works even when expired! I've used Abreva for about 4 years now and it's worked for me every time I needed to use it, which is about once a year.
Everyone here saying "just go to your doctor and get prescription stuff", it is really that easy to make an appointment? I needed to make my annual physical 4 months in advance for clinics that's covered under my insurance. I probably can do walk-in clinics, but then I'll have to wait for who knows how long and pay the office visit copayments. These are not be as good as prescription stuff, but comparing efficacy of OTC with prescription doesn't seem like a fair comparison as one requires more steps to obtain.
Abreva works but lemon balm tinctures taken in water are far more effective in my experience. Taking the tincture as soon as you feel the slightest onset of a breakout has stopped my breakouts 9 out of 10 times.
I need to add I take the tinctures 2-3 times per day for a few days after the initial dose.

My company has a corporate owned HMO with a clinic located at our building. I can pretty much walk in anytime during the weekday 8-4 hours though I sometimes need to set appointments which are a day or two at most.
I used to have Kaiser Permanente and it was fairly reasonable scheduling here. A week at most.
I suspect your HMO has oversold service in your area.
Next time you go to Europe, walk into any pharmacy and ask for this type thing. It's stronger and effective and over the counter. Put it on early and next day it's gone. It's cheaper than Abreeva and a bigger tube
Vectavir, Fenavir, and other well-known brands are widely available as topical creams over-the-counter (OTC) in European and Scandinavian pharmacies . They contain Penciclovir 1%, the identical active antiviral ingredient in Denavir cream. It helps prevent HSV I & II from multiplying, thereby shortening the outbreak. Denavir does indeed cost around $400 (or more!) for a tiny tube in the US, and is still expensive mail order (with a valid prescription) from Canada too. It is inexplicable that it is not similarly available in the US.

A general tip to people is to make sure you wash your face at least twice a day. I used to be lazy and only wash face when I showered. But washing every night before sleep I think helps alot .

Here is what I do and don't do.
I don't take Lysine as a daily vitamin.
When a cold sore appears I immediately crush a lysine tablet into a powder, mix it with a tiny amount of water and apply the paste to the sore. At the same time I ingest 3000 mg of the Lysine vitamin. Then Every hour or so I apply additional paste to the sore. Usually next morning the sore is either gone (depending on how quickly I caught it) or it hasn't gotten any larger than it was when I first applied the paste. For me Stress is the sole reason cold sores appear. Years ago Dr told me I could take a daily oral medication to prevent cold sores but he didn't think it was a good idea. I agreed.
