New wording for Metro unlock policy:

The device must be purchased from Metro by T-Mobile.
The device must not be reported as lost, stolen or blocked.

Should obviously only apply to purchases made after they changed the rules.
But might have to fight for that...

For me when I click on the phone,
I see the choice of $19.99 for new number
and $0.00 for keeping old number.
After you click on one of the choices, you click on Add to Cart button.

Metro's website is very bad.

I have an account with them and the line is still active.

I cannot add another line.

Most current deals require a port-in AND a plan that's eligible for the discount. If you don't see the discounted price with a $40 plan, try selecting the $65 ($60 with Autopay) plan.

For an iPhone, you can go to www.sickw.com , enter its IMEI and use the free iPhone SIM Lock function to look up its unlock status. Sickw simply querries that Apple database. You can't check the unlock status of an Android phone because they're not unlocked like an iPhone.

Different values depending on whether you're trading (one phone) directly toward a purchase or trading in for an Apple GC. You get a lower amount when trading in for a GC, but you can use multiples GCs toward a purchase.
