I have a legacy Simple Choice plan so this deal made it slick for me. I also opted for the iPhone 14 Plus which was $8.34/mo + tax. The agent I spoke to was able to waive their activation fee and expedited 2 day shipping for free. You'll eventually have to call T-Mobile to get the deal but you can price out your options online. Here are the steps I took.
Quick note: Both phones require you pay the monthly charge for 24 months.

Select your storage size and color. 256Gb is an extra $100.
Select 'Existing Customer' (Log into your account)
Select 'Keep my number'. You'll need a number to port in. I used a Google Voice number ($3 to unlock).
Select 'Skip Trade-in'
You should see the promotion applied and a monthly payment of $8.34 + tax. Tax will need to be paid upfront which was about $50.
Last note: I was also able to qualify for a free line but I'm sure this is a targeted promotion. It wouldn't hurt to ask if you're also eligible. In the end, I got a free line and a cheap iPhone 14 Plus.

It looks like the minimum of $2 is for the wearable lines such as an Apple Watch line.
The $5 is for actual cell phone lines
I have simple choice, so far no texts of a price change on my lines.
